  Please provide your name, mobile number and date of birth, along with your current address including the neighborhood, and your current occupation.

• Body info and area

  Height · Weight

  Enter your height and weight and select the surgical area you want. If you are seeking revision surgery, please also select 'Revision'.

• Six body photos

  Front · Back · Side

  Full-body photos from six directions are required: face, front, back, side, right 45° and left 45°.

• 360° Video

  One full turn

  Please also submit a 360° video of one full turn on the spot. It shows the silhouette that photos alone make hard to assess.

• History

  Tattoos · Surgery

  Please state accurately whether you have tattoos, your history of plastic surgery and procedures, and your medical history. Applications for liposuction revision surgery cannot be accepted.

• SNS and Motivation

  Along with your SNS account URL, please write your motivation for applying and what you can do for TheLINE as a Real Model.

1. APPLICATION

   Application Submission

   Fill out and submit the application form below. Please send your full-body photos, 360° video and motivation through the channel we guide you to after submission.

2. DOCUMENT REVIEW

   Document Review

   We review the submitted photos, video and history to confirm whether you are suitable for the surgical area you requested.

3. CONSULTATION & EXAM

   In-Clinic Consultation and Examination

   If you pass the review, we contact you individually. You visit the clinic for a consultation with a specialist and an examination to confirm whether surgery is possible.

4. FINAL SELECTION

   Final Selection and Consent Form

   Once you are finally selected, we go over the scope of photography and review usage together, and you complete the consent form for portrait rights and review usage.

5. SURGERY & PHOTOS

   Surgery and Progress Photography

   After surgery we photograph and record your progress in line with each stage of recovery. Photography schedules are adjusted according to your recovery condition.

* If you are selected as a Real Model, your before-and-after photos, videos and review may be used on the clinic website, on social media and in advertising media. The scope and duration of use are stated in the consent form, and no material is used without your written consent.

* Surgical results may vary from person to person.
