STEP 1 · Assessing the eye area
The amount and position of under-eye fat, the depth of the tear trough and the skin thickness are assessed precisely.

Tear Trough Correction
This surgery suits those whose under-eye fat makes them look tired or gives a dark impression. Protruding or hollowed fat tissue is repositioned into a smooth, even surface for a brighter overall impression.
Under Eye Fat Repositioning
TheLINE under-eye fat repositioning is a surgery that brings a dull under-eye area back to a fresh, bright look.

It suits those whose under-eye fat makes them look tired or gives a dark impression. Protruding or hollowed fat tissue is repositioned into a smooth, even surface for a brighter overall impression.
When under-eye fat pushes forward, the area just below it looks relatively hollow, and the step between them casts a shadow. Because the shadow comes from contour rather than pigment, lasers or makeup rarely erase it.
The fat is accessed through a micro-incision on the conjunctiva inside the eyelid. No incision line is left on the outer skin, so there is less concern about a visible scar.
If the fat is only taken out, the hollow tear trough stays as it is. At TheLINE, the protruding fat is not discarded but moved into the hollow to fill it, evening out the entire under-eye area.
Recommendation Target
Transconjunctival under-eye fat repositioning is recommended for the following cases.
Fat protrusion
Cases where under-eye fat pushes forward so the area looks puffy or uneven.
Tired look
Cases where dark circles make you look constantly tired or give a dark impression.
Contour, not pigment
Cases where the shadow remains even after laser treatment for dark circles, making improvement difficult.
Thin under-eye skin
Cases where the under-eye skin is thin enough that the vessels beneath show through.
Aged appearance
Cases where a deepening tear trough makes you look older than your actual age.
Surgery Steps
TheLINE proceeds in four steps, from assessing the eye area to finishing the surgery.
The amount and position of under-eye fat, the depth of the tear trough and the skin thickness are assessed precisely.
A micro-incision is made in part of the conjunctiva inside the eyelid to reach the fat layer.
The protruding fat is moved down to the lower eyelid, into the hollow tear trough, and spread out evenly.
As the step-off disappears the under-eye area flattens out, leaving a clearer, younger-looking eye contour.
Why TheLINE Eye Surgery
TheLINE, a clinic that has worked with fat for years, handles under-eye fat.
REPOSITION NOT REMOVE
We do not stop at removing the protruding fat. Only by moving it into the hollow tear trough does the step-off disappear and the under-eye area even out.
NO VISIBLE SCAR
Because the approach goes through the conjunctiva inside the eyelid, no incision line is left on the outer skin. The operation time is short, around 30 minutes, with same-day discharge.
ANESTHESIOLOGIST ON SITE
Sedation and local anesthesia are managed by a Board-certified Anesthesiologist & Pain Medicine Specialist, with real-time monitoring on university-hospital-grade anesthesia equipment. Pre-operative testing is carried out as well.
SKIN LAXITY CARE
If the under-eye skin is loose, repositioning alone may not be enough. In that case we also consider lower blepharoplasty, which includes removing the sagging skin (about 1 hour, local anesthesia, 1 week of recovery).
PROVEN EXPERIENCE
Since opening in 2007, we have worked with liposuction, fat grafting and SVF for 19 years. Hands that know how fat behaves handle under-eye fat well too.
