What "dissolving" actually means

Fat-dissolving injections are described as dissolving fat. It is a figure of speech, but it does not stray far from what actually happens.
A fat cell is close to a sac enclosed by a thin membrane, with oil-based contents inside. When the injected agent breaks down the structure of that membrane, the sac ruptures and the contents come out.
The released fat does not simply disappear. Immune cells gather to clear it, and it is metabolized and excreted through the blood and lymph. That is why the change does not appear immediately but progresses over time.
This is also why the area does not shrink right after the injection but instead swells for a few days: an inflammatory response occurs as part of the clearing process.
The ingredient called deoxycholic acid
Among the ingredients used in fat-dissolving injections, the one formally approved in Korea is deoxycholic acid. It is also the ingredient approved by the US FDA for fat reduction.
The name is unfamiliar, but it is a substance the body already contains. It is a type of bile acid whose role is to break down the fat we eat during digestion. In effect, its fat-handling property is put to direct use.
The approved indication is submental fat. Clinical studies reported that when treatment was given at one-month intervals, submental fat decreased in a substantial proportion of participants and that state was maintained for a certain period.
That said, in Korea it is not uncommon for clinics to mix various ingredients at their own discretion and use them under the name of a fat-dissolving injection. It is worth checking what is actually being injected.
How does it differ from liposuction?
Both reduce the number of fat cells. But the scale and the procedure are completely different.
Liposuction is surgery. Anesthesia is involved and a cannula is inserted to physically remove fat. A large amount can be handled in one session and the contour can be designed.
Injections are a non-surgical procedure. They are done with about the level of numbing cream and return to daily life is quick. In exchange, the amount that can be handled at once is small, so it is carried out over several sessions.
So what they target is different. Injections are not suited to reducing volume across a broad area. They come up for narrow areas such as under the chin or above the knees, or for refining fine irregularities remaining after liposuction.
A plan to substitute repeated injections for surgery is generally inefficient.
| Category | Fat-dissolving injection | Liposuction |
|---|---|---|
| Classification | Non-surgical procedure | Surgery |
| Anesthesia | About the level of numbing cream | Anesthesia is involved |
| Amount handled per session | Small, so it is carried out over several sessions | Large, and the contour can be designed |
| Target areas | Narrow areas such as under the chin or above the knees, and fine irregularities left after liposuction | Volume across a broad area |
How TheLINE divides injections from surgery
In consultation, TheLINE Plastic Surgery Clinic first looks at the scale of the volume. How much has to be reduced is what divides an injection approach from a surgical one.
For narrow areas, or when surgery feels like too much, starting with a fat-dissolving injection or a liposuction injection is an option. Conversely, if the area is broad, liposuction is more efficient than repeating injections.
TheLINE does not recommend injections as a substitute for surgery. If repeated sessions do not reduce the area as much as hoped, both time and cost increase. Among TheLINE patients, some hear this explanation during consultation and change their plan.
Dr. Jaeho Cho and Dr. Yoosuk Chung set the direction after checking the area and its condition.
Where is it used?
The most frequently mentioned area is under the chin. A double chin does not involve a large amount of fat yet is highly visible, which makes it a good fit for an injection approach.
On the body it is used in narrow, protruding spots such as above the knees, the inner arm, and in front of the armpit. These are the areas that catch on clothing.
It is also used to refine fine irregularities remaining after liposuction. The order is to address the bulk surgically and then adjust the finer details.
Conversely, the cases where it does not fit are clear. Little effect can be expected on broad areas such as the whole abdomen, on an abdomen caused by visceral fat, or on areas where the issue is muscle or sagging skin rather than fat.
It also differs from liposuction injections
The similar name that causes confusion is the liposuction injection. Its purpose overlaps with the fat-dissolving injection, but the approach differs.
With a fat-dissolving injection, the agent breaks down the membrane of the fat cells. After the cells rupture, you wait for the body to clear them.
With a liposuction injection, an agent is introduced to liquefy the fat and it is then drawn out on the spot. Because the clearing is not left to the body but done directly, it sits a step closer to surgery.
Which one fits depends on the area, the thickness, and the recovery period you can accommodate. TheLINE Plastic Surgery Clinic positions SlimSlim Suction (Bbaera-Bbaera) and the Ssak Injection for different situations.
Rather than choosing by name alone, it is better to check what the procedure actually does and how.
Recovery and progress
The area swells right after the procedure, because an inflammatory response occurs as the fat cells break down and are cleared. Swelling often continues for several days to about a week.
Pain, a sensation of heat, and bruising can appear alongside. Parts that feel firm to the touch may develop and soften over time.
Changes are confirmed gradually. Since the clearing and excretion process runs on a scale of weeks, it is hard to judge right after a single session.
Carrying it out over several sessions at intervals is standard. The number of sessions varies by area and condition, so it is decided during consultation.
Limits worth knowing
It does not replace weight loss. It is a procedure for refining narrow areas, so its effect on overall body weight is minimal.
Individual variation in results is large. Responses differ with the thickness and character of the fat and with metabolic condition. The same number of sessions does not produce the same degree of change.
If the skin is sagging, extra caution is needed. As fat decreases, the slack skin can become more apparent. In that state a lift is reviewed alongside.
Side effects are possible as well. Swelling, pain, bruising, nodules, and reduced sensation can occur, and rarely the skin surface is left uneven. Unexpected inflammation or bleeding may occur, and the degree varies from person to person, so caution is required.
Checking which ingredient is used matters too. Please ask during consultation whether it is a formally approved ingredient and for which area that approval applies.
Frequently Asked Questions
Does one session work?
It is not a procedure that finishes in one session. Since the clearing and excretion of fat runs on a scale of weeks, carrying it out over several sessions at intervals is standard.
Can I just repeat injections instead of liposuction?
The amount that can be handled per session is small, so efficiency is low for broad areas. If repetition does not reduce the area as much as hoped, both time and cost increase.
The injected area feels firm. Is that alright?
Parts that feel firm can develop as the fat is cleared, and they usually soften over time. However, if pain worsens or the area reddens, it needs to be checked.
If I gain weight again, does the area come back?
Removed cells do not regenerate, but the remaining cells can enlarge. If body weight increases significantly, that area changes along with it.
How does TheLINE divide injections from surgery?
TheLINE Plastic Surgery Clinic first looks at the scale of the volume to be reduced. Narrow areas and fine post-surgical adjustments are approached with injections, and for broad areas we advise that liposuction is more efficient.
